Get ready for RMHP and D-SNP plan changes in 2026

Due to federal rating rule changes, Rocky Mountain Health (RHMP) Medicare Advantage and Dual Special Needs (D-SNP) plans will end Dec. 31, 2025, in Western Colorado.

Available plans
Through 2025 and beyond, RMHP and UnitedHealthcare will continue to serve Dual Eligible (Medicare and Medicaid) members in Western Colorado through two UnitedHealthcare Dual Complete Plans: CO-S4 (HMO-POS D-SNP) and CO-S002 (HMO-POS D-SNP).

The available D-SNP plans will be managed by RMHP, but use the UnitedHealthcare D-SNP provider network, instead of the previously available RMHP network.

RMHP contracts do not apply to these plans.

Next steps
If you’re already contracted with the UnitedHealthcare D-SNP network you’ll be automatically included for 2026. Please verify the following Medicare and D-SNP network IDs are listed in your Provider Express secure portal account. Go to:

• Select My Network Status
• Choose My Networks
• Verify that your network IDs include Medicare and CO Dual Enrolled D-SNP
