Form I-9
Applicable to staff of Optum New Mexico only.
Because your legal employer is changing, federal law requires you to complete a new Form I-9 (Employment Eligibility Verification).
It’s a good idea to start gathering the documents you’ll need to complete your Form I-9 now. See the Lists of Acceptable I-9 Documents below. You’ll need to provide original, unexpired documents and present these documents to an authorized approver between Dec. 14 and Dec. 18.

Form I-9 Deadline
Please complete Section 1 and have your manager or designated approver review your eligibility documentation and complete Section 2 of your Form I-9 between
Dec. 14-18.
Per federal law and UnitedHealth Group policy, your employment will be terminated if Section 1 and Section 2 of your Form I-9 are not completed by Thursday, Dec. 24 at 2 p.m. MT.
