We’re working to advance health equity by identifying and addressing system inequities, eliminating barriers to care, and supporting a modernized Medicaid program that brings together community-based organizations to address Social Determinants of Health and deliver culturally relevant care. Our highly coordinated services address physical and behavioral health needs, as well as social determinants of health such as food, clothing, shelter and transportation.

We work to ensure our members can rely on a health care system that is accessible and connected. This includes the delivery of high-quality care that is responsive to the needs of each member, as well as the communities in which they live. We work to remove barriers that stand in the way and to empower our members to be active participants in their own health.
