We focus on programs and practices that deliver proactive, integrated care that addresses not just health conditions, but also the social and environmental factors related to an individual's well-being. This approach is designed to reduce duplication of services and treatments that only address a portion of the underlying health need – ultimately curbing costs while also improving outcomes.
Developing partnerships and resources to address areas that directly affect health and quality of life

Bringing together local care providers, community leaders and neighborhood social programs builds trust and helps entire communities strengthen their physical, social and economic health. A key component of our work involves strong collaboration with providers. We work alongside providers, listening to their needs and gathering insights about how we can help simplify and enhance their experience. We also focus on enhancing providers’ ability to deliver high-quality care by developing value-based care programs and investing in supports and technology that help expand their capacity and capabilities.

UnitedHealthcare Community Plan of Texas was awarded the Health Equity Accreditation by the National Committee for Quality Assurance (NCQA) for meeting or exceeding rigorous requirements for health equity.
